INTRODUCTION
Accidents are not only disruptive to work but also costly affairs to the company. Whilst the incidence of accidents at the workplace is on the rise, employers need not resign to the fact that these accidents are beyond their control. Employee education on safety awareness as well as improving work environment may effectively prevent industrial accidents in the workplace. Critically all incidents should therefore be thoroughly investigated demonstrating a proven company-wide process of efficiency and effectiveness linked to the Safety Management System (SMS). The more serious the incident the more extensive the investigation hence in this respect, this training will benefit employees and safety & health committee members.
TRAINING OBJECTIVES
At the end of this training course, the participants will be able to:
- Be aware of legal and regulatory requirements related to accident /incident reporting and investigation.
- Grasp the importance of accident/ incident investigation in maintaining a safe work environment.
- Learn the significance of identifying root causes in preventing recurrence of incidents.
- Understand and apply the concept of Root Cause Analysis (RCA) by exploring various RCA methodologies combine with 5 Whys Methodology.
- Familiarize participants with the step-by-step process of incident and accident investigation.
- Learn how to document accidents/ incidents accurately and comprehensively.
- Learn how to develop and implement effective corrective/ preventive actions.
- Emphasize the importance of learning from accidents/ incidents (lesson-learnt)/ for continuous improvement.

METHODOLOGY
This stimulating program will maximize the understanding and learning through interactive lectures sessions, group discussion, video presentation and practical activities.
